Recovery Care

1 – 6 weeks after an injury or procedure

  • Overseeing daily nutrition, hygiene, and rest

  • Implementing doctors’ recommendations

  • Transportation to/from appointments and errands

  • Movement and exercise support

Continuous Care

Ongoing care suited to changing needs

  • Home coordination (daily agenda and calendar management)

  • Support with emails, voicemails, and phone calls

  • Overseeing daily nutrition, hygiene, and rest

  • Movement and exercise support

  • Transportation to/from appointments and errands

  • Advocating at appointments by asking about risks/benefits and notetaking

  • Family communication

End-of-Life Care

Diagnosis through end of life

  • On-call support

  • Holding space and creating comfort during transition

  • Educating family members on end-of-life signs and symptoms

  • Helping clients communicate wishes and say goodbye

  • Working alongside hospice care to ensure that needs are met in all areas

Additional Services

May be added on to care packages

  • Weekly shower only

  • Medication management

  • Completion of the Five Wishes health directive, which ensures that aging clients are able to clearly communicate their wishes to loved ones

  • Assistance with local and online wellness and education resources related to health conditions and/or lifestyle preferences

Customized Trainings

Intensive education for groups and families

With the increase of individuals receiving life-changing diagnoses earlier in life, Cori is now offering private, customized group trainings on how to better support a loved one facing health changes (such as dementia) as they age. After the training is complete, families will have the option of scheduled check-ins at regular intervals. To learn what this training may look like for you, please schedule a consultation.

Working with Navigating Elderhood

– We are flexible. 

– Services are available from two hours up to a full day.​

– We create a set schedule while knowing that changes may occur as life and death happen. 

– We work virtually around the world and in-person in the Twin Cities, MN.

– Fees are based on support services needed and are evaluated or adjusted as applicable. 

– Services can be given as gifts.

Packages can be combined and expanded as wants and needs change. Services do not include managing finances, decluttering houses, or cooking large meals.
